I regret to inform you that Advanced Center for Infertility and Reproductive Medicine, RPC (ACIRM) and its affiliated laboratory (CPRL) and ambulatory surgical facility (Center for Reproductive Surgery, LLC) are all permanently closed due to my health concerns.

If you are a former patient of ACIRM and have embryos or sperm stored for future use at a cryo-storage facility, all of the laboratory information required by any fertility center is with the sperm and embryos at that cryo-storage facility. Your entire medical record has been transitioned to Shady Grove Fertility Centers. If you elect to seek your care elsewhere, your ACIRM records may be retrieved from Shady Grove Fertility Centers for that purpose.

If you are a former patient of ACIRM, with the understanding that your fertility issue may not yet be resolved or that you may want to attempt to have another child, your medical records have been transitioned to Shady Grove Fertility Centers. If you elect to seek your care elsewhere, your ACIRM records may be retrieved from Shady Grove Fertility Centers for that purpose.

To retrieve your ACIRM records, Shady Grove Fertility Centers can be reached at 888-761-1967. If you have difficulty retrieving your records, or you find the cost prohibitive (infertility records are often many hundreds of pages, and Shady Grove Fertility Centers cannot be expected to discern which records are pertinent to copy on your behalf), please do not despair. It is very likely that the center at which you shall receive your future care will need to repeat much, if not all, of the testing procedures that are addressed in your records, in the course of your pursuit of additional fertility care.
